THERAPY FEES

Initial intake session: $175

60-minute individual session: $150

I believe strongly in people’s right to have access to high quality mental healthcare, regardless of their ability to pay. It is for this reason that I offer a limited number of reduced fee sessions for clients who are uninsured, “under-insured,” or who are experiencing financial hardship. Sliding scale fees are determined on an individual basis and will be discussed and decided upon together during our initial consultation.

INSURANCE

I am currently an in-network provider with BlueCross BlueShield of Illinois PPO and BlueChoice plans, as well as the Northwestern University NU-SHIP Aetna plan. If I am not in-network with your insurance, I am happy to provide you with an invoice to send to your insurance company for partial reimbursement. If you plan to use insurance, I require this information prior to the first session so that I may check your coverage and let you know if you have a copay. I accept cash, all major credit cards, and personal checks as methods of payment for services. Payment is expected at the end of each session.

QUESTIONS FOR YOUR INSURANCE COMPANY

When you contact your insurance company, it can be very helpful to come equipped with specific questions to ask about your coverage.

  • Does my policy include outpatient mental health benefits?

  • If so, do I have a deductible to meet before my benefits start?

  • Is my mental health deductible part of, or separate from, my medical deductible?

  • How much of my deductible have I met for this year?

  • What percentage of services will you reimburse once my deductible is met?

  • What is my co-payment or co-insurance per session?

  • Is pre-authorization or pre-certification required? If yes, how many sessions are authorized before a treatment plan is required?

  • Are there any coverage or session limitations?

  • Do I have out-of-network benefits for outpatient mental health appointments?

  • Do I have a deductible to meet before my benefits start?

  • What is the process to submit for out-of-network reimbursement?

APPOINTMENT CANCELLATION

If you need to cancel your appointment, I require a minimum of 24-hours notice; otherwise, you will be charged $95 for the missed appointment. This is because insurance companies do not reimburse for missed or cancelled appointments. Messages may be left on my voicemail or email and will accurately record the date and time that you called or messaged me.
